Peter D. Carlon Endowed Student Leadership Award

Requirements:
The Peter D. Carlon Endowed Student Leadership Award shall be benefit of the Department of Kinesiology in UT Arlington. Funds distributed from the endowment shall be used to support an annual student award or awards, which will recognize a graduate athletic training student in the Department of Kinesiology who has established him- or herself as a leader within the Athletic Training Program and has demonstrated a commitment to professional and/or community service.

  2. Please submit a one page essay, double spaced, describing your current and future contributions to the Department of Theatre Arts and Dance.

  4. Identify and explain one experience during your academic preparation or clinical rotations that has allowed you to demonstrate leadership.

  6. What does leadership mean to you? Explain any leadership experiences that will aide in your career as an athletic trainer.
  7. Have you identified a faculty member that will complete the Kinesiology Scholarship Recommendation Form? The faculty member will have access to the form.
  8. Please submit a name and email address of one of your professors you would like to invite to complete the recommendation form on your behalf. Once this information is entered, a link will be sent to your professor to complete. Be sure to notify them they will receive an email requesting to complete the recommendation form and ask that they provide it prior to the scholarship deadline.
